About the role

Responsibilities
- Perform comprehensive patient assessments, formulate differential diagnoses, and develop evidence-based treatment plans.
- Order and interpret diagnostic tests, prescribe medications within scope, and manage acute and chronic conditions.
- Coordinate care across multiple settings, ensuring clear communication with patients, families, and interdisciplinary team members.
- Accurately document encounters in the electronic health record (EHR) in compliance with regulatory standards.
- Incorporate patient education, preventive care, and health promotion to empower active patient engagement.
- Participate in quality improvement initiatives and contribute to optimizing clinical workflows and patient experiences.
Qualifications & Skills
