Nurse Practitioner

About the Role

As a hybrid role, you will split your time between on-site visits with patients and remote work from home, supported by a dedicated scribe team for efficient documentation. Your success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct on-site visits and remote monitoring to detect abnormalities and prevent hospitalizations
• Collaborate with our clinical team to develop and implement individualized care plans
• Ensure seamless integration with the facility's processes and protocols
• Demonstrate excellent communication, collaboration, and teamwork skills

Requirements

• Active Nurse Practitioner license (in the state of practice)
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
