About the role
At Carease Health - NJ, we are committed to prioritizing proactive measures for enhanced physical health and well-being through our comprehensive preventative care program. We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team in Haskell, NJ.
Responsibilities
- Provide comprehensive geriatric or internal medicine care to residents in skilled nursing facilities
- Conduct on-site visits and remote work from home, with support from a dedicated scribe team
- Build strong relationships with facility staff and maintain a patient-centered approach
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
- Develop and implement individualized care plans for patients
- Maintain patient progress and adjust treatment plans as needed
Requirements
- Active NP or PA license in the state of practice
- 1+ year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
- Masters degree in Nursing or related field
