Position Summary Blue Ridge Care is seeking a compassionate and experienced Nurse Practitioner to provide primary care services within our PACE program. Under the supervision of the Medical Director, this role performs comprehensive assessments, manages ongoing care, and partners closely with the Interdisciplinary Team (IDT) to support frail elderly participants. This position plays a vital role in developing care plans, treating acute and chronic conditions, and providing education to participants and caregivers while ensuring high-quality, coordinated care. Essential Duties And Responsibilities Perform comprehensive history and physical assessments for new participantsConduct semi-annual and as-needed reassessments and update plan of careCollaborate with the IDT to develop and implement individualized care plansEvaluate and treat participants during acute and chronic illnessCoordinate referrals to specialists in collaboration with the Primary Care PhysicianMaintain accurate and timely participant medical records and documentationProvide education to participants, caregivers, and staffParticipate actively in IDT meetings and provide clinical inputCoordinate care across multiple care settings and external providersSupport quality improvement initiatives and identify care enhancementsServe as a clinical resource to team membersEnsure compliance with all regulations, policies, and safety standardsMaintain confidentiality and follow infection control proceduresParticipate in continuing education and required trainingsPerform other duties as assigned Education And Experience Graduate of an accredited school of professional nursing requiredLicensed as a Registered Nurse (RN) and Nurse Practitioner in the stateCertification as a Nurse Practitioner by a nationally recognized organizationGeriatric Nurse Practitioner (GNP) certification with prescriptive authority preferredMinimum of 2 years of experience working with frail or elderly populations requiredExperience in a multidisciplinary setting preferred (hospital, nursing home, or community-based)2+ years of experience as a Nurse Practitioner in geriatrics preferredCurrent CPR certification requiredValid driver’s license, reliable transportation, and proof of insurance requiredMust meet immunization and communicable disease requirements Skills And Knowledge Strong understanding of geriatric and chronic care managementKnowledge of home- and community-based careStrong communication and interpersonal skillsAbility to work collaboratively in an interdisciplinary teamStrong organization and time management skillsAbility to manage changing priorities in a dynamic environmentProficient in EMR systems and general computer useAbility to read, write, and speak English fluently Work Environment & Physical Requirements May be exposed to healthcare-related risks (bodily fluids, communicable diseases, etc.)Ability to stand, walk, bend, and lift up to 50 poundsAbility to push/pull up to 150 pounds with proper equipmentRequires manual dexterity and coordinationWork environment may be fast-paced, noisy, and emotionally demanding Your Benefits Health, Dental & Vision InsuranceRetirement Plan with Company MatchPaid Time Off (PTO)Paid Volunteer TimeThrift Shop DiscountVoluntary Benefits (including Critical Illness) Why Blue Ridge Care?
