Nurse Navigator RN

Job Profile Summary

This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. It also involves performing Nurse Navigator duties such as guiding patients through the healthcare system and overcoming obstacles to their care.

Job Description

• Manages projects and processes independently with limited supervision.
• Resolves difficult and complex problems.
• Coordinates the transition of care from one health care setting to another, including inpatient, home health care, skilled nursing facility, and rehab facilities.
• Educates patients and families about their clinical conditions, treatments, and recovery.
• Collaborates and communicates with multidisciplinary providers to achieve exceptional patient experiences and outcomes.

Minimum Qualifications

• Associate's degree in Nursing.
• Registered Nurse (RN) license.
• Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification.
• Three (3) years clinical experience caring for patients with a broad range of complex medical diagnoses.
• Valid, state-issued driver’s license and reliable transportation.

Preferred Qualifications

• Bachelor's degree in Nursing.
• Advanced Practice Registered Nurse (APRN) eligibility for state and federal controlled substances registration.
• Five (5) years clinical experience in home health, post-acute, or acute settings.
• Bilingual.

Duties and Responsibilities

• Uses effective relationship management, coordination of services, resource management, education, patient advocacy, and related interventions to improve quality of care and prevent rehospitalization.
• Promotes cost-effective nursing, medical, and functional outcomes.
• Collaborates with multidisciplinary providers to achieve exceptional patient experiences and outcomes.

Physical Requirements

• Prolonged, extensive, or considerable standing/walking.
• Lifts, positions, pushes, and/or transfers patients and equipment.
• Considerable reaching, stooping, bending, kneeling, and crouching.
• Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise, and possible exposure to radiation, lasers, electric shock, etc.
• Regularly exposed to the risk of bloodborne diseases and other transmissible infections.
• Contact with patients under various circumstances.
• Subjected to varying and unpredictable situations.
• Handle emergency and crisis situations.
• Subjected to irregular hours.
• May have contact with hazardous materials.

Skills & Abilities

• Effective communication with patients, families, physicians, and healthcare team.
• Knowledge and application of the nursing process to implement a nursing plan of care.
• Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.
• Knowledge of growth and development; ability to interpret age-specific data and responses to care; and provide age-appropriate communication.
• Maturity, self-confidence, and ability to follow complex patient management plans in collaboration with medical staff.
• Ability to provide primary care for a caseload of patients according to practice guidelines and hospital policies, procedures, and protocols.

Pay Range

$84,300.22 - $107,481.20
