Nurse Navigator
About the role
Facilitates communication between the patient, their primary care physician and specialists to improve clinical outcomes.
Works collaboratively with the patient to foster self-management and compliance to their clinical plan of care.
Works collaboratively with physician, hospital and community resources to support the patient's clinical plan of care.
Responsibilities
- Education: Bachelor's Degree in Nursing Required
- Master's Degree in Nursing Preferred
- Experience: 3 years experience in disease or case management services with focus on telephonic management, medical reconciliation and ambulatory care coordination
- Required General Experience with clinical, hospital-based information systems
- Licenses: PA Registered Nurse License Required, Multi State Compact RN License Required
