Minimum qualifications
- Bachelor’s degree in Nursing
- Current State of Michigan Registered Nursing License
- CPR or BCLS Certified
- Three years of experience in an acute care setting
- One year of experience in medical, surgical or radiation oncology (for oncology specialty)
What the Nurse Navigator will do
Completes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and functional status in collaboration with multidisciplinary team members.
Includes assessment of past and present treatment course.
Affirms and develops goals, treatment plan and provider options in concordance with evidence based standards and clinical trials.
Works directly with physician lead to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of care.
Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for patients.
Uses critical thinking and problem solving skills to analyze and synthesize data and formulate an appropriate case management plan to address the patient’s individual and family needs.
