About the role
The Nurse Navigator coordinates care across the continuum for patients who need follow-up appointments after diagnostic mammography and low-dose lung screening exams. They facilitate timely appointments and communication between providers.
Responsibilities
- Facilitate timely follow-up appointments for patients after diagnostic mammography and low-dose lung screening exams.
- Work collaboratively with other healthcare team members to improve patient care, experience, and outcomes.
- Serve as a link to community services to assist patients in accessing necessary resources.
Requirements
- Registered Nurse license in Oklahoma.
- BLS certification.
Qualifications
- 4-year Bachelor's Degree in Nursing or related field.
