Nurse Navigator

About the Role

This Nurse Navigator integrates healthcare for pediatric patients, collaborating with a multidisciplinary team to coordinate care, ensure timely treatment, and support families by removing barriers.

Responsibilities

• Facilitate patient care across settings
• Promote early intervention
• Evaluate outcomes using clinical and utilization data
• Serve as a key communication link among providers, clinics, and external facilities
• Utilize independent nursing judgment and adhere to age-specific care protocols
• Work closely with support services
• Problem-solve and improve processes
• Work in both structured and flexible environments

Qualifications

• Bachelor's degree in Nursing preferred; required in certain locations
• Accredited nursing program diploma
• At least two years of clinical experience preferred
• Active RN license in the practicing state
• Ongoing competency and certification requirements
