About the Role
The Nurse Navigator provides care coordination services to patients, families, and post-acute care providers, focusing on acute and chronic conditions, including women's health and pregnancy.
Responsibilities
- Facilitates interdisciplinary collaboration among care teams.
- Coaches patients and caregivers in care planning and self-management.
- Assesses patient understanding of disease processes and reinforces discharge instructions.
- Collaborates with internal and external care teams, including physicians and managed care partners, to ensure seamless service delivery and optimal outcomes.
- Maintains comprehensive patient records and evaluates care processes.
- Addresses care issues and coordinates obstetric care.
- Schedules consults and imaging appointments.
- Manages referrals and documentation.
- Supports educational initiatives for patients and families.
- Works within the Fetal Care Center, training and cross-covering with other nurse navigators.
Requirements
- Bachelor’s degree in Nursing or related field.
- Valid RN license in Michigan.
- At least two years of clinical and care coordination experience.
- Experience with process improvement methodologies like Lean is preferred.

- Ability to facilitate care transitions and patient education.
