About the role

This position provides telephonic and back-office nursing support to a primary care practice by answering chronic care questions, managing prescription refills and prior authorizations, and communicating lab results to patients. The Nurse Navigator coordinates closely with referral specialists and assigned providers, offering consistent support to a defined patient panel. While primarily desk-based and phone-focused, the role occasionally assists in direct patient care by helping medical assistants room patients when the clinic is busy. Responsibilities

  • Answer chronic care questions from patients via telephone and provide clear, compassionate, and accurate guidance.
  • Serve as a primary telephonic point of contact for patients, managing a call volume that can range from approximately 70 to 400 calls per month.
  • Process prescription refill requests efficiently and in accordance with clinical protocols and regulatory requirements.
  • Complete and manage prior authorizations for medications, procedures, and services to ensure timely patient access to care.
  • Communicate lab results to patients, explain findings in an understandable way, and reinforce provider care plans.
  • Coordinate with referral specialists as needed to support continuity of care and ensure appropriate follow-up.
  • Document all patient interactions, clinical information, and care coordination activities accurately in the eClinicalWorks (eCW) electronic medical record system.
  • Collaborate closely with assigned providers, including physician assistants, nurse practitioners, and physicians, to support their patient panels and care plans.
  • Assist medical assistants with rooming patients and direct patient care tasks on an as-needed basis, typically up to approximately 10% of the role.
  • Support primary care management by contributing to care plans, chronic disease management, and ongoing patient education.
  • Adapt to a fast-paced environment and manage changing priorities while maintaining high-quality patient care and service.
  • Handle interactions with a challenging patient population with professionalism, resilience, and strong communication skills.

Requirements

  • Active Missouri (MO) nursing license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
  • Experience working in a telephonic or call center nursing role, or similar high-volume patient communication environment.
  • Proficiency with electronic medical record systems, specifically eClinicalWorks (eCW).
  • Experience in primary care, including experience with care plans and chronic disease management.
  • Demonstrated ability to manage prescription refills and prior authorization processes.
  • Strong communication skills for explaining lab results, care plans, and clinical information to patients in a clear and empathetic manner.
  • Ability to work effectively with providers, including physician assistants, nurse practitioners, and physicians, in a team-based care model.
  • Resilience and strong emotional composure, with the ability to maintain professionalism when working with a difficult or complex patient population.
  • Ability to adapt quickly to changing needs and situations in a fast-paced clinical environment.

Additional Qualifications

  • Experience in primary care or outpatient clinic settings.
  • Prior experience in a back-office nurse or nurse navigator role.
  • Familiarity with care coordination and referral processes.
  • Comfort providing limited direct patient care, including assisting medical assistants with rooming patients when needed.

Work Environment

This is a full-time, weekday position with schedules typically running Monday through Friday, with start times between 8:00 a.m. and 9:00 a.m. and end times between 4:00 p.m. and 5:00 p.m. The role is primarily desk-based and telephonic, working in a shared space alongside medical assistants and using the eClinicalWorks (eCW) electronic medical record system throughout the day. The environment is fast-paced and highly collaborative, with a close-knit team that values mutual support and genuinely cares about patients and colleagues. The position may offer hybrid flexibility, with occasional in-clinic responsibilities that include limited direct patient care such as helping room patients when the clinic is busy. The culture emphasizes work-life balance with no weekend shifts.

Pay

The pay range for this position is $26.00 - $45.00 per hour.

This is a Contract to Hire position based in Kansas City, MO, and is fully onsite.
