About the role
The Nurse Navigator Acute Care is a multi-faceted position providing comprehensive services to patients throughout the acute care continuum. Working in the roles of registered nurse, case manager, and patient/physician liaison, this position is responsible for the clinical, quality, and cost-effective outcomes for a caseload of patients. This role requires the advanced knowledge and expertise to manage cardiovascular hospitalized patients, understand triage principles, and identify and implement improvement processes.
Responsibilities
- Manage cardiovascular hospitalized patients
- Understand triage principles
- Identify and implement improvement processes
- Collect, trend, and analyze aggregate outcomes data for the patient population
- Identify best practices and target performance improvement efforts
- Work with nursing staff and management, physicians, patients, families, community agencies, and professional peers
Requirements
- Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service may be considered in lieu of education
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
- Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
- Current BLS Certification (if direct patient contact)
- Certification in assigned area preferred
- Three (3) recent clinical experience in area of specialty
Pay and Benefits
Pay expectations: Competitive salary, medical, dental, and retirement benefits, and paid time off. The pay scale is $109,006.56 - $177,821.28 USD. Associated benefit information can be viewed here.
