About the role
The Nurse Navigator Acute Care is a multi-faceted position providing comprehensive services to patients throughout the acute care continuum. This role involves working as a registered nurse, case manager, and patient/physician liaison.
Responsibilities
- Manage clinical, quality, and cost-effective outcomes for a caseload of patients.
- Collect, trend, and analyze aggregate outcomes data for the patient population to identify best practices and target performance improvement efforts.
- Manage cardiovascular hospitalized patients with advanced knowledge and expertise.
- Understand and apply triage principles.
- Identify and implement improvement processes.
Requirements
- Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service may be considered in lieu of education.
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred.
- Current Registered Nurse license in Washington State or Multistate License endorsement (MLS).
- Current BLS Certification (if direct patient contact).
- Certification in assigned area preferred.
- Three (3) recent clinical experience in area of specialty.
Pay and Benefits
Competitive salary, medical, dental, and retirement benefits, and paid time off are provided. The pay scale is $112,674.00 - $197,579.20 USD. Associated benefit information can be viewed here.
