Nurse Manager - Continuum of Care

This leadership position oversees clinical operations, ensuring the delivery of safe, high-quality patient care across multiple locations. Responsibilities include implementing best practices, developing care delivery systems, resolving patient concerns, and collaborating with healthcare professionals to optimize outcomes. The role involves monitoring resource utilization, maintaining equipment and supplies, fostering a safe environment, and providing clinical supervision and delegation within scope of practice. Additionally, the role manages team development, performance improvement, and operational workflows, aligning efforts with organizational goals.

Responsibilities

• Lead team development through coaching, training, and performance feedback; promote collaboration and professional growth.
• Manage clinical teams and workflows by translating strategic plans into actionable tasks, ensuring policy adherence, and resolving escalated issues.
• Oversee care operations by supporting compliance, resource management, data analysis, and stakeholder partnerships to ensure seamless patient care across the continuum.
• Drive process improvements by identifying root causes, implementing solutions, and monitoring performance metrics to enhance quality and efficiency.

Qualifications & Skills

• Bachelor’s degree in Nursing (BSN) with at least three years of experience in healthcare operations or related fields.
• Minimum two years in a leadership role, with experience in customer or patient service.
• Licensed Registered Nurse in relevant states, with required credentials from multiple states; Basic Life Support certification.
