Nurse Manager - Continuum of Care

Responsibilities
- Develops and mentors staff, promotes collaboration, and drives performance improvement through coaching and feedback.
- Manages team workflows, ensures policy adherence, resolves escalated issues, and builds accountability for results.
- Oversees care at home operations, ensuring compliance, managing human resources, and supporting compliance readiness and budget management.
- Ensures standardized care delivery by developing partnerships, managing complex projects, and monitoring operational activities for seamless patient transitions.
- Leads process improvements by identifying root causes, implementing solutions, and aligning performance metrics with strategic initiatives.
Qualifications
Bachelor’s degree in Nursing (BSN) with at least three years in healthcare operations or related fields. Minimum two years in a leadership role, with experience in customer or patient service. Must hold or obtain multi-state nursing credentials, including RN licenses from designated states and Basic Life Support certification.
