Responsibilities

  • Planning, directing, coordinating, and evaluating the personnel activities within defined areas of practice to ensure the provision of quality patient care 24 hours a day.
  • Contributing to the strategic planning process and attainment of goals of Charter House.
  • Promoting ongoing development of staff and maintaining a professional environment in which all staff grow and develop.
  • Clinical leadership on the unit, serving as a role model, mentor, and clinical resource for nursing staff.
  • Supervising the nursing care provided by professional and ancillary nursing personnel.
  • Applying the nursing process to assess resident needs, develop, implement, and update individualized care plans, implement evidence-based interventions, and evaluate outcomes to promote functional ability, safety, and quality of life in accordance with Minnesota Department of Health (MDH) and applicable federal regulations.
  • Collaborating with the interdisciplinary team, residents, and family/significant others to coordinate services across the continuum of care.
  • Utilizing clinical and non-clinical software systems to maintain accurate records, and contributing to a culture of quality, safety, and regulatory readiness.

Qualifications

  • A BSN from an accredited school of nursing, or an associate degree in nursing and 3 years of long-term care experience.
  • Licensure/Certification: Current RN license registered with the Minnesota Board of Nursing and CPR certification.
  • Master's degree in nursing, business, or a health-related field is preferred.
  • Previous nursing experience with long-term care, including dementia care, assisted living, home health, or acute care is preferred.
  • Long-term care experience is preferred.
  • Ability to work independently.
  • Good communication and human relations skills to interact with staff, residents, and families/significant others.
  • Strong problem-solving skills, and the ability to collect, compare, sort, and prioritize information to be used in the decision-making process for groups of residents and employees.
  • Adaptable to unpredictable situations and stress.
  • Maintain current CPR certification.

  • Evaluation must be completed by an organization listed as a member of the National Association of Credential Evaluation Services (NACES) (www.naces.org) or the Association of International Credential Evaluators, Inc. (AICE) (www.aice-eval.org).
