Duties and Responsibilities
- Prevent abuse, neglect, or exploitation of aged and disabled adults.
- Serve as court-appointed guardian for incompetent adults.
- Assess daily needs and provide case management to Medicaid residents of Adult Care Homes needing enhanced personal care services.
- Provide in-home services to aged and disabled adults to help them remain safely in their homes.
- Maintain oversight of Adult Care Homes and provide guidance and support to individuals considering opening new Adult Care Homes.
- Offer case management to aged and disabled adults receiving Adult Day Care services.
- Monitor the operations of Adult Day Care centers.
- Handle intake and provide information and referrals for all Adult Services.
- Provide transportation for Medicaid recipients to all Medicaid-covered medical appointments.
Knowledge, Skills And Abilities
- Thorough knowledge of social work principles, techniques, and practices.
- Considerable knowledge of medical, behavioral, and psychological problems and their treatments.
- Considerable knowledge of family and group dynamics and intervention techniques.
- Considerable knowledge of governmental and private organizations and resources in the community.
- Considerable knowledge of relevant laws, regulations, and policies.
- General knowledge of casework supervision and training.
- Knowledge of medical terminology, disease processes, and appropriate therapies based on medical or psychological diagnosis.
- Ability to establish rapport with clients and apply assessment techniques.
- Ability to communicate effectively and plan work efficiently.
Desired Education and Experience
- Bachelor's degree from an accredited school of social work or a four-year degree in a human service field with at least 15 semester hours in related courses.
- One year of social work experience preferred.
