Duties and Responsibilities

  • Prevent abuse, neglect, or exploitation of aged and disabled adults.
  • Serve as court-appointed guardian for incompetent adults.
  • Assess daily needs and provide case management to Medicaid residents of Adult Care Homes needing enhanced personal care services.
  • Provide in-home services to aged and disabled adults to help them remain safely in their homes.
  • Maintain oversight of Adult Care Homes and provide guidance and support to individuals considering opening new Adult Care Homes.
  • Offer case management to aged and disabled adults receiving Adult Day Care services.
  • Monitor the operations of Adult Day Care centers.
  • Handle intake and provide information and referrals for all Adult Services.
  • Provide transportation for Medicaid recipients to all Medicaid-covered medical appointments.

Knowledge, Skills And Abilities

  • Thorough knowledge of social work principles, techniques, and practices.
  • Considerable knowledge of medical, behavioral, and psychological problems and their treatments.
  • Considerable knowledge of family and group dynamics and intervention techniques.
  • Considerable knowledge of governmental and private organizations and resources in the community.
  • Considerable knowledge of relevant laws, regulations, and policies.
  • General knowledge of casework supervision and training.
  • Knowledge of medical terminology, disease processes, and appropriate therapies based on medical or psychological diagnosis.
  • Ability to establish rapport with clients and apply assessment techniques.
  • Ability to communicate effectively and plan work efficiently.

Desired Education and Experience

  • Bachelor's degree from an accredited school of social work or a four-year degree in a human service field with at least 15 semester hours in related courses.
  • One year of social work experience preferred.
