Responsibilities
- Provide supervision, coaching, mentorship, and clinical guidance to Nurse Care Managers and MDS Assessment Nurses.
- Monitor staff productivity, performance, assessment quality, and documentation compliance.
- Conduct regular supervision meetings, case consultations, and performance evaluations.
- Support recruitment, onboarding, training, and professional development of nursing staff.
- Ensure timely completion and quality of MDS assessments and post-discharge assessments in accordance with CCA requirements.
- Oversee medication reconciliation processes and compliance with nursing and care management standards.
- Promote person-centered, trauma-informed, and culturally responsive care practices.
- Serve as a clinical resource for complex cases involving medical, behavioral health, substance use, and social determinants of health needs.
- Support staff in managing high-acuity individuals and overcoming barriers to care and engagement.
- Monitor care plans to ensure they reflect individual goals, interdisciplinary input, and integrated care approaches.
- Utilize data and performance metrics to evaluate program effectiveness and improve outcomes.
- Implement strategies that reduce preventable emergency department visits, hospitalizations, and healthcare costs.
- Ensure compliance with CCA, MassHealth, and organizational standards, including documentation, audit readiness, and quality requirements.
- Participate in quality improvement initiatives, audits, and corrective action planning.
- Collaborate with interdisciplinary teams, providers, community partners, and funding sources to support coordinated care.
- Aid in the implementation of new workflows, policies, systems, and program requirements.
- Support transitions of care and provide direct assistance during staffing shortages, high-volume periods, or complex care situations.
- Participate in leadership meetings, trainings, committees, and professional development activities.
- Respond to emails and phone communications within one business day.
Qualifications
- Bachelor of Science in Nursing (BSN) required.
- Active Registered Nurse (RN) license in Massachusetts required.
- Minimum of 5 years of clinical experience in care management, case management, or community-based healthcare required.
- Minimum of 2 years of supervisory or leadership experience required.
- Experience working with individuals with complex medical, behavioral health, and social needs required.
- Strong knowledge of care management models, interdisciplinary care teams, and social determinants of health.
- Experience supervising, mentoring, and developing clinical staff.
- Familiarity with electronic health records (EHRs), email systems, and healthcare technology.
- Strong verbal and written communication skills.
- Knowledge of community resources and care coordination practices.
- Valid driver's license and reliable, legally registered vehicle available during working hours required.
- Experience with One Care, MassHealth, and/or Commonwealth Care Alliance (CCA) preferred.
- Familiarity with MDS assessments, care planning requirements, and quality compliance standards preferred.
