About the role

Location: Keizer, Oregon. Facility: Keizer Court Post Acute of Cascadia Healthcare, a leading skilled nursing and rehabilitation center committed to providing exceptional care in a safe, comfortable, and welcoming environment. Schedule: Full‑time and part‑time onsite positions. Shift details: RN (full‑time) – Thursday–Sunday, 2 pm–10 pm; LPN (full‑time) – Tuesday, Thursday, Friday, 2 pm–10 pm; LPN (part‑time) – Sunday, Monday, Wednesday, 2 pm–10 pm. We’re seeking a dedicated and detail‑oriented Nurse (LPN or RN) to join our team and help us deliver exceptional clinical care and support to residents and their families.

Responsibilities

• Provide direct nursing care and clinical oversight in accordance with care plans, physician orders, and company policies.
• Supervise and support nursing staff to ensure safe and effective resident care.
• Administer medications and treatments accurately and in compliance with regulations.
• Assess, monitor, and document resident conditions, communicating changes promptly to the physician and care team.
• Participate in developing and updating individualized care plans.
• Maintain accurate clinical records and ensure documentation meets all facility, state, and federal standards.
• Respond to medical emergencies and provide immediate nursing interventions as needed.
• Collaborate with interdisciplinary teams to promote resident well‑being and recovery.

Requirements

• Education: Graduate of an accredited LPN or RN School of Nursing.
• Licenses/Certification: Current valid LPN or RN license in the state of employment; valid CPR certification.
• Experience: Six months of experience in a long‑term care environment preferred.
• Additional: Must pass state‑specific background check prior to contact with patients/residents.
