Nurse Home Visitor - NFP

About the role

Deliver compassionate, relationship-centered care to first-time parents in their homes and communities through regular nurse home visits starting during pregnancy and continuing until the child’s second birthday. This role combines clinical nursing practice with coaching, education, and care coordination.

Responsibilities

• Provide education, guidance, and support to first-time parents to foster lifelong health and stability.
• Collaborate with a multidisciplinary team to ensure comprehensive care and long-term impact.
• Work within the Nurse-Family Partnership (NFP) model to empower families.

Requirements

• Bachelor's degree in Nursing or related field.
• Current licensure as a Registered Nurse (RN).
• Experience in home visiting or similar direct care setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
