Nurse Home Visitor

About the Role

The Nurse Family Partnership under our subsidiary is a program that provides comprehensive services to first-time mothers and their newborn children. This program aims to improve the lives of families and children experiencing poverty and adversity. Our specially trained nurses work closely with mothers and children to assist them through this stage of their lives.

Responsibilities

• Make home visits to mothers enrolled in the Nurse Family Partnership (NFP) program.
• Provide care coordination and basic services such as blood pressure monitoring and weighing the child.
• Document case visits for billing and record-keeping.
• Network with community resources and the medical community to integrate the program into local resources.
• Interact with and direct the efforts of external stakeholders.

Qualifications

• Bachelor of Science in Nursing or Associate Degree in Nursing.
• Active Alabama license as a Registered Nurse (RN) in good standing.
• BLS CPR/AED Certification.
• Experience in a position exercising independent judgment and decision-making.
• Experience in a position demonstrating exceptional written, verbal, and interpersonal communication skills.
• Valid Alabama Driver’s License and ability to travel daily as required.

Preferred Experience

• Experience working in public/community health, including home visitation.
• Experience providing maternal/child health services, particularly in a community setting.
• Experience working with diverse populations, communities, and organizations.
