NFP Nurse - Family Partnership Home Visitor

About the role

This role involves providing case management and support services as a Registered Nurse (RN) Home Visitor within the Nurse-Family Partnership Program.

Responsibilities

• Conduct home visits with pregnant and parenting women and their children
• Collaborate with community agencies
• Ensure compliance with regulatory standards, policies, and protocols
• Deliver holistic care addressing physical, psychological, social, educational, and safety needs across all ages
• Demonstrate professionalism and teamwork
• Adhere to organizational policies
• Promote research and performance improvement
• Provide comprehensive patient care

Qualifications

• BSN obtained within the last five years
• Current RN licensure
• BLS certification
• Minimum of two years’ recent experience in pediatrics or related fields involving children birth through five years
• Experience in adolescent psychology, obstetrics, home care, or community nursing is preferred

Skills

• Strong communication skills, especially with adolescents
• Valid driver’s license with insured vehicle
