NFP Nurse - Family Partnership Home Visitor

About the role

This position is for a Registered Nurse (RN) Home Visitor responsible for delivering case management and support services to pregnant and parenting women and their children under a research-based program.

Responsibilities

• Conduct home visits to provide care and support.
• Collaborate with community agencies to coordinate services.
• Ensure compliance with regulatory standards and program policies.
• Address the physical, psychological, social, educational, and safety needs of patients.
• Adhere to all program protocols and demonstrate professionalism.
• Participate in performance improvement activities.
• Deliver high-quality patient care within a team setting.

Requirements

• Valid RN license.
• BLS certification.
• BSN preferred (required within 5 years if not already obtained).
• At least two years of recent pediatric or related experience with children birth through five.
• Strong communication skills, especially with adolescents.
• Valid driver’s license and insured vehicle.

Schedule

This is a part-time, onsite position.
