About the role
The Nurse Coordinator will work with the Bone Mineral & Metabolism Clinic healthcare team, staff, and other healthcare providers to provide excellent transitions of care within the healthcare setting.
Responsibilities
- Coordinate patient care
- Develop procedures and provide training/education
- Collaborate with the multidisciplinary team
- Utilize clinical outcomes data to support innovative and cost-effective changes to clinical practice
- Accountable to patients and families for the processes and outcomes of care during an episode of care
- Accountable to peers for the advancement of group practice in the practice setting
- Accountable to the nurse/clinic manager for the totality of work performance
- Participate in practice setting governance
Requirements
- Bachelor of Science in Nursing (BSN)
- Registered Nurse (RN) license
- National certification in area of specialty, ambulatory care nursing, or care coordination and transition management within 2 years
