About the role

The Nurse Coordinator will work with the Bone Mineral & Metabolism Clinic healthcare team, staff, and other healthcare providers to provide excellent transitions of care within the healthcare setting.

Responsibilities

• Coordinate patient care
• Develop procedures and provide training/education
• Collaborate with the multidisciplinary team
• Utilize clinical outcomes data to support innovative and cost-effective changes to clinical practice
• Accountable to patients and families for the processes and outcomes of care during an episode of care
• Accountable to peers for the advancement of group practice in the practice setting
• Accountable to the nurse/clinic manager for the totality of work performance
• Participate in practice setting governance

Requirements

• Bachelor of Science in Nursing (BSN)
• Registered Nurse (RN) license
• National certification in area of specialty, ambulatory care nursing, or care coordination and transition management within 2 years

• 4 years of related experience

Schedule
