Job Requirements
- Education: Associate Degree of Nursing, Graduate from a college or university accredited by NLNAC or CCNE
- Experience: Possess one of the following certifications: CCM, CDMS, CRRN, COHN, ACCC, CMC, RN-NCM, or CCM
- Experience: Minimum of 3 years (ADN) or 2 years (BSN) full-time experience in clinical case management with diverse populations. Familiarity with the military healthcare system and telehealth experience is desirable. Work experience with Tri-care and Military Health System Genesis system preferred.
- Additional certifications: Maintain current certification in BLS. Additional certifications such as ACLS, PALS, and NRC as required by AFI 44-102 or equivalent.
Key Responsibilities
- Comprehensive Case Management: Provide expert case management for military members, veterans, and their families across various healthcare settings, focusing on acute, chronic, and complex health issues.
- Patient Advocacy: Serve as a dedicated advocate for military members, ensuring they receive the highest standard of care, while addressing their physical, emotional, and psychosocial needs.
- Collaborative Care Coordination: Work seamlessly with interdisciplinary healthcare teams to coordinate and integrate care across all levels and sites, facilitating a holistic approach to patient well-being.
- Military Community Engagement: Engage with the military community, fostering relationships with service members and their families, and contributing to the unique healthcare challenges faced by those serving in the armed forces.
- Innovative Healthcare Solutions: Contribute to the development and implementation of innovative healthcare solutions, including telehealth initiatives, to enhance accessibility and quality of care for military members.
