Responsibilities
- Perform medical case management, screening, education, counseling, and medication management.
- Network with community healthcare providers to coordinate care.
- Participate in a rotating on-call schedule, primarily via remote phone consultations, ensuring accessible support.
Qualifications
- BSN or ASN from an accredited program.
- Current Illinois RN license and driver’s license required.
- Experience with psychiatric care and children/adolescents is preferred.
- Strong organizational, communication, and clinical skills.
