About the Role
As a Nurse Case Manager, you will support youth in emotional, psychological, and physical care within a residential setting.
Responsibilities
- Medical case management
- Screening, education, and counseling
- Medication management
- Networking with community healthcare providers
- Participating in a rotating on-call schedule, primarily via remote phone consultations
- Provide comprehensive health services and ensure accessibility via remote consultations
- Collaborate with community providers
Requirements
- Bachelor or Associate Degree in Nursing
- Current Illinois RN licensure
- Strong organizational, communication, and clinical skills
- Experience with psychiatric care or children/adolescents is preferred
