About the role
Mentors new or lower level case managers, facilitating their growth and development in the role and ensuring their success in case management.
Responsibilities
- Facilitates the growth and development of new or lower level case managers.
- Ensures the success of case managers in their role.
- Performs independently and requires a broad understanding of principles and practices.
- Performs additional clinical tasks and responsibilities that may be performed or undertaken by an RN, including but not limited to:
- Assessment of critical need or high risk patients to ensure appropriate level of care is prescribed and provided.
- Participation in case management programs that require clinical assessments.
Requirements
- Bachelor's Degree in Nursing.
- 12 months of professional nursing experience.
- Will accept an Associate's degree in Nursing and 24 months of professional nursing experience in lieu of the Bachelor's Degree for a total of 36 months professional nursing experience.
- State of Oklahoma Registered Nurse Certification.
- One year of paid professional experience in direct service provision, care planning, and has provided services to the aging and/or disabled population.
- Certified in Case Management.
- Experience in a home-like setting.

Pay
- Pay Range: Targeted salary up to $60,500, based on experience.
Schedule
- Work Schedule: Monday – Friday Standard / Fixed Schedule.
Travel
- Required.
