Nurse Case Manager

About the role

We are seeking a compassionate and experienced Nurse Case Manager to join our team. In this role, you will serve as a key advocate and care coordinator, developing and managing comprehensive care plans to promote the health and well-being of our patients.

Key Responsibilities

• Develop, implement, and revise individualized patient care plans in collaboration with patients, families, physicians, and healthcare teams.
• Act as the primary point of contact and advocate for patients and their families, facilitating access to medical providers and community resources.
• Provide essential health education to patients and families to improve understanding and management of chronic conditions.
• Conduct thorough assessments to identify unmet health and social needs.
• Ensure medication adherence and compliance with follow-up appointments through ongoing support and communication.
• Maintain timely and accurate documentation and communicate effectively with all members of the care team regarding patient status.

Qualifications

• Required:
• - Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois.
• - Minimum of 3 years of nursing experience.
• - Minimum of 2 years of experience with geriatric patients.
• - Ability to commute to and work on-site in Oakbrook, IL.
• - 2+ years of experience in Case Management or Care Coordination.
• - Experience in post-acute or long-term care settings within the last 5 years.
