Non-exempt position, reporting to the Charge Nurse/Nursing Manager.
Pay
$32–$44 hourly, based on experience.
Requirements
- Active Massachusetts nursing license.
- Bachelor’s degree in human services or related field from an accredited college or university.
- At least one year of recent, relevant professional experience working with adults in a case-management capacity.
- Minimum one year of direct-service experience with the elderly or chronically disabled.
- Minimum one year of direct-service experience with individuals who have memory loss and/or dementia.
- Demonstrated ability to plan and lead health-related activities.
- Strong communication, organizational, and documentation skills.
- Sensitivity to the needs of elders from diverse socioeconomic backgrounds and ability to collaborate across disciplines.
Responsibilities
Clinical/Nursing
- Develop, monitor, implement, and evaluate individualized treatment and care plans.
- Participate in ongoing health-status monitoring through direct contact with physicians, other nurses, health professionals, and family members.
- Carry out physician’s orders: monitor vital signs, administer medications, check blood sugar, implement treatments, and coordinate follow-up with caregivers and physicians.
- Implement and supervise restorative-therapy treatments recommended by physical, speech, and/or occupational therapists.
- Assist, monitor, and instruct participants, program assistants, and staff on personal hygiene, use of special aids, accident prevention, and ambulation/transferring skills.
- Supervise program assistants who provide personal care and daily-task support.
- Provide health education and training on both an individual and group basis, including scheduled monthly clinical groups.
- Document initial health assessments, care plans, monthly nursing notes, MDS, and reassessment forms in conjunction with the Program Manager.
Case Management/Social Services
- Serve as primary advocate for participants and caregivers, coordinating services to meet social, emotional, and health-related needs.
- Develop, implement, and update individualized social-service care plans, progress notes, and discharge plans as required by regulations.
- Coordinate services with other agencies, caregivers, and health providers to ensure continuity of care.
- Conduct family and care-planning meetings; participate in interdisciplinary and clinical meetings.
- Provide counseling, crisis intervention, and behavioral support as needed.
- Assist participants and families in accessing benefits and community resources when no other advocate is available.
- Lead or co-lead group discussions, caregiver-education sessions, and support groups.
- Assist with admission functions: referral management, tours, guest days, and eligibility assessments in collaboration with nursing staff.
General Program
- Participate in interdisciplinary case conferences and weekly clinical meetings.
- Assist participants with Activities of Daily Living (ADLs) in accordance with care plans.
- Maintain current knowledge of community resources and regulatory requirements.
- Ensure all care and documentation meets DPH and DMA standards.
- Work collaboratively with the Program Director and team to ensure quality of care, participant satisfaction, and program compliance.
- Perform other related duties as assigned to meet program needs.
