About the role
Provide comprehensive nursing oversight in adult residential group homes for medically complex individuals. The Nurse Case Manager ensures optimal care and safety for residents, manages health assessments, and supports staff through training and audits. The role involves collaboration with interdisciplinary teams to coordinate care plans, monitor outcomes, and maintain compliance with regulatory and quality standards.
Responsibilities

- Oversee nursing needs across multiple group homes
- Dedicate approximately 8–10 hours per home weekly
Requirements
- BSN required
- A minimum of 3 years of clinical nursing experience
- Experience with the Special Needs population highly preferred
- Current CT RN License in good standing
Qualifications
- Bachelor of Science in Nursing (BSN)
- Minimum of 3 years of clinical nursing experience
- Experience with Special Needs population preferred
- Current Registered Nurse (RN) license in good standing in Connecticut

Pay
Retail market pay for the position.
Schedule
Monday through Friday, 8am - 4:30pm - weekend on-call rotation.
