Nurse Care Manager- Full Time

Nurses: Are you looking to focus on care and quality of life; not paperwork? Martha’s Hands is expanding our team of Nurse Care Managers for Private Pay Home Care Clients! Martha's Hands Nurse Care Managers utilize your nursing skills as part of a 12‑person Care Supervisory Team to guide clients and families through care needs, support and train caregivers in providing direct care in the home, and partner with home health, hospice, and other care professionals to manage care.

Responsibilities

• Guide a client and their family through their care needs
• Support and train caregivers in providing direct care to clients in their homes
• Partner with home health, hospice, and other care professionals to manage care
• Conduct assessments with clients and families to develop a customized service plan
• Coordinate care with other health care professionals including home health, hospice, primary care, etc.
• Orientate and train caregivers to provide excellent care
• Provide clinical oversight of clients and support to caregivers
• Provide direct care to clients, performing skilled nursing tasks and ADL assistance as needed (medication management, wound care, diabetic care, tube feedings, etc.)
• Assist families with more detailed care management services as needed
• Balance office and mobile work environments, utilizing technology to enhance care and equip nurse care managers to be more efficient and effective
• Supervise the care team to ensure quality care expectations are met while inspiring and encouraging caregivers

Requirements

• 3+ years of nursing experience
• Ideal experience in private duty, home health, hospice, geriatric nursing, or med‑surg
• Team approach to care utilizing strong professional and interpersonal communication skills
• Strong clinical assessment skills to develop a customized service plan that directs client care
• Educational and training abilities to enhance caregiver success in caring for clients
