NP Provider Hospice & Palliative Care (Pediatric)

About the role

This role involves providing pediatric hospice and palliative care services in community and occasional hospital or long-term care settings.

Responsibilities

• Conduct in-person or telehealth visits
• Assess patients and develop care plans
• Coordinate with the healthcare team
• Support training and contribute to quality improvement initiatives
• Ensure care aligns with best practices and evidence-based protocols

Requirements

• Master’s degree in nursing
• NYS Nurse Practitioner license (Pediatric, Family, or Palliative Care)
• CPR/BLS certification
• Pediatric or hospice experience preferred
• Certification in hospice & palliative care encouraged within two years
