About the Role
Provide in-person and telehealth services to hospice and pediatric palliative care patients in community, hospital, or long-term care settings.
Responsibilities
- Deliver comfort care to patients.
- Conduct assessments and develop care plans.
- Coordinate with the interdisciplinary team.
- Support team training initiatives.
- Contribute to quality improvement projects.
Requirements
- Master's degree in Nursing and graduation from an approved Nurse Practitioner program.
- NYS Nurse Practitioner license (Certified Pediatric, Family, or Palliative Care).
- CPR/BLS certification.
- Pediatric Nurse Practitioner experience preferred; prior hospice or palliative care experience advantageous.
- Strong communication, teaching, organizational, and critical thinking skills.
- Ability to work flexibly, travel as needed, and collaborate within a team environment.
- Encouraged to obtain Hospice & Palliative Care certification within two years.
- Must possess a valid driver’s license and vehicle.
