About the Role
A Pediatric Nurse Practitioner (NP) specializing in hospice and palliative care, delivering community-based services across multiple locations.
Responsibilities
- Deliver in-person and telehealth hospice and palliative care, including assessments, care planning, treatment, and documentation.
- Coordinate with team physicians to evaluate hospice eligibility and manage urgent medical needs.
- Support training and development of the interdisciplinary team.
- Contribute to quality improvement projects and develop pediatric hospice care curricula.
Requirements
- Master’s degree in nursing.
- NP certification (pediatric, family, or palliative care).
- Current New York State license.
- CPR/BLS certification.
- Valid driver’s license.
- Prior pediatric NP or hospice experience preferred.
- Certification in hospice and palliative care encouraged within two years of hire.
