NP Provider - Hospice & Palliative Care (Pediatric)

Role Overview: This field-based Pediatric Nurse Practitioner (NP) in Hospice/Palliative Care provides patient-centered care across Manhattan, Bronx, Brooklyn, Queens, and Long Island. Key Responsibilities: Offer clinical services to hospice-enrolled patients, including in-person and telehealth visits. Conduct assessments, develop care plans, and coordinate with interdisciplinary teams. Support the training of hospice staff, contribute to quality improvement projects, and assist in pediatric palliative care consultations. Qualifications & Skills: Master's in Nursing and graduation from an approved NP program are required. Preferred candidates have pediatric NP experience, hospice or palliative care background, and NYS NP licensure (Pediatric, Family, or Palliative). Additional requirements include CPR/BLS certification, a driver’s license, and willingness to travel. Strong communication, organizational, critical thinking, and cultural competency skills are essential. Certification in Hospice & Palliative Care is encouraged within two years. Key Duties: Deliver compassionate, evidence-based care, support interdisciplinary teams, and engage in community and occasional long-term or hospital-based consultations to enhance end-of-life and pediatric hospice services.
