Job Summary

The Nurse Practitioner or Physician Assistant will provide high-quality primary care and support services to individuals and families within the local community. This position is a full-time, day-shift role, Monday through Friday, from 8:00 a.m. to 5:00 p.m. The provider will work collaboratively with physicians, nurses, and other healthcare professionals to deliver patient-centered care in a community health setting.

Responsibilities

  • Conducting comprehensive health assessments
  • Diagnosing and managing acute and chronic conditions
  • Developing individualized treatment plans
  • Providing preventive care services such as immunizations, screenings, and health education
  • Managing follow-up care
  • Cooking referrals to specialists when necessary
  • Ensuring continuity of care for patients
  • Maintaining accurate and timely documentation in electronic health records
  • Adhering to clinical protocols and regulatory requirements
  • Participating in quality improvement initiatives

Qualifications

  • Education Required: RN with a master's degree in nursing certification in family and/or adult nurse practitioner.
  • Experience Required: Previous Experience In a FQHC Strongly Preferred.
  • Position Requirement(s): License/Certification/Registration Required - Current licensure to practice nursing in the State of Tennessee.
  • Preferred Certification in areas of specialty.
