NP/PA - Senior Health

About the role

The Senior Health & Wellness Center (SHWC) is an outpatient geriatrics clinic providing primary and consultative care for adults aged 65 and older. Nurse practitioners serve as primary care providers and consultants to meet the needs of an expanding older adult population.

Responsibilities

• Serve as a primary care provider and consultant for older adults
• Offer comprehensive care including care management, counseling, diabetes education, foot care, geriatric specialty consultations, memory and cognitive evaluations, medication education, and chronic disease management

Requirements

• NP Board Certification (AGNP, ANP, GNP, or FNP) or PA Certification (NCCPA)
• Oregon NP or PA license (or ability to obtain during credentialing)
• Certification in BLS, ACLS or PALS required within 30 days of hire

Qualifications

Experience in geriatric medicine is preferred but not required.

• Ability to work collaboratively with a multidisciplinary team
