About the role

Ascension Seton is looking for an APRN or PA for our Ascension Seton Health Clinics. This is a great opportunity for the right candidate to provide medical services to patients in their community. The Post-Acute and Long Term Care Program provides nursing home visits in the rural community at 2-3 sites. Travel is required. Practice setting is outpatient with a shared patient panel. This is an employed position within a nonprofit health system.

• Lead Comprehensive Care Delivery: Direct the treatment and long-term management of diverse health conditions, ensuring every patient receives a personalized and effective care plan.
• Empower Patients and Families: Translate medical jargon into actionable guidance, educating caregivers on treatment options and community resources to improve health literacy.

Requirements

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required (American Heart Association or American Red Cross accepted).
• Nurse Practitioner credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.
• Master's degree in Nursing required.
