About the role
This is an exceptional opportunity for a motivated advanced practice provider to join an employed position within a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network. Join 2 other dedicated providers within a comprehensive, multidisciplinary team utilizing a daily round-table model to coordinate patient care.
Responsibilities
- Flexible Schedule & Shared Call: Benefit from a predictable Monday through Friday daytime schedule, with a rotating call schedule split evenly among the 3 providers
- Practice Setting: Inpatient/Outpatient mix, providing on-site comprehensive primary care within the PACE center, as well as periodic clinical rounding at three contracted skilled nursing and short-term rehab facilities when required
- Enhance Patient Access: Manage the specialized needs of an adult population (aged 55+) with complex medical and social conditions, empowering individuals to safely remain in their homes rather than transitioning to traditional nursing home care
- Employed position within a nonprofit health system
- Board Certified or Board Eligible in Nurse Practitioner or Physician Assistant
- Eligible for state medical licensure and DEA registration
- Drive Clinical Excellence: Deliver proactive, longitudinal health outcomes through structured initial assessments, 6-month reassessments, and stabilized chronic disease management, while maintaining acute/urgent care capabilities using dedicated on-site bays (IV fluids, EKGs, point-of-care testing)
- Compassionate Continuity: Confidently manage advanced chronic conditions and guide critical end-of-life or hospice care scenarios when they arise
Requirements
- Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Nurse Practitioner obtained prior to hire date or job transfer date required.
- Registered Nurse obtained prior to hire date or job transfer date required.
- Education: Master's degree of Nursing required.
