NP/PA- Community Health Center (Extended Hours)

Full-time, day-shift role with 3-10 hour shifts from 8:00am–7:00pm.

About the role

The Nurse Practitioner or Physician Assistant will provide high-quality primary care and support services to individuals and families within the local community. This role involves a team-based approach to care, working collaboratively with physicians, nurses, and other healthcare professionals to deliver patient-centered care in a community health setting.

Responsibilities

• Conduct comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized treatment plans.
• Provide preventive care services such as immunizations, screenings, and health education to promote wellness and disease prevention.
• Manage follow-up care, coordinate referrals to specialists when necessary, and ensure continuity of care for patients.
• Maintain accurate and timely documentation in electronic health records, adhere to clinical protocols and regulatory requirements, and participate in quality improvement initiatives.
• Educate patients about their health, collaborate with interdisciplinary teams, and foster trust within the community.
• Address social determinants of health by connecting patients to appropriate resources and support services.
• Develop evidence-based protocols and utilize them to create individualized plans of care.
• Collect, document, and analyze patient data; collaborate with the healthcare team in testing and treatment implementation.
• Participate in the development and implementation of health screening programs for adolescents and adults, and develop protocols for identifying high-risk individuals.
• Participate in community and corporate education on internal medicine topics.
• Demonstrate knowledge of growth and development principles across the lifespan and assess data to meet age-specific patient needs.

Requirements

• RN with a master’s degree in nursing and certification as a family and/or adult nurse practitioner.
• Graduate of a nationally accredited school of nursing.
• Current licensure to practice nursing in the State of Tennessee; eligibility to sit for certification in area of specialty.
• Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non-Tennessee residents.
• Previous experience in a Federally Qualified Health Center (FQHC) strongly preferred.
• Preferred certification in areas of specialty.
