About the Role

This position involves providing in-home medical care within assigned counties, collaborating with interdisciplinary teams to support patient health and well-being.

Responsibilities

• Conduct comprehensive assessments of patients
• Develop and update care plans
• Perform basic nursing procedures
• Educate patients and their families on disease management and wellness
• Coordinate with healthcare providers
• Conduct follow-up assessments
• Participate in quality improvement initiatives to ensure high standards of care
• Practice according to state laws and maintain current medical knowledge through continuing education

Requirements

• Current RN/NP license or ability to obtain it
• Certification from AANP or ANCC
• Active DEA licensure or ability to obtain
• Reliable transportation and valid driver’s license
• Adherence to safety policies and minimum employment duration

Qualifications

• Preferred: 2+ years clinical NP experience in primary or urgent care settings
• Strong organizational and communication skills
• Computer literacy
