About the Role
Provide in-home clinical care as part of an interdisciplinary team, focusing on high-risk patients within their residence.
Responsibilities
- Conduct comprehensive assessments, including medical history, vital signs, and physical exams.
- Manage acute episodes and chronic condition exacerbations.
- Provide patient and caregiver education and counseling.
- Document thoroughly and communicate effectively with the care team.
- Participate in advanced care planning and evidence-based screenings.
- Collaborate with team members to deliver holistic, preventive care that promotes patient well-being at home.
Requirements
- Active licensure and certification (e.g., ANCC, AANP, NCCPA) in relevant fields.
- Unrestricted state license and DEA prescriptive authority.
- Reliable transportation and ability to navigate varied environments.
Preferred Qualifications
- 1+ years of clinical experience.
- Geriatric or home health background.
- Proficiency with EMRs.
- Experience managing complex patient needs.
