Role Overview

This position involves providing in-home care to high-risk patients, focusing on comprehensive assessments, managing acute and chronic conditions, and collaborating with an interdisciplinary team. The role emphasizes patient education, advanced care planning, and documentation, with a focus on holistic, preventive care to help patients stay well at home.

Responsibilities

• Conducting thorough in-home health assessments, including medical history, vitals, and screenings.
• Managing health issues during acute episodes and sub-acute rehabilitation.
• Communicating effectively with team members and documenting care accurately.
• Engaging in advanced care planning and providing patient and caregiver education.

Qualifications & Skills

• Active certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (ANCC or AANP).
• Unrestricted licensure and driver’s license in the applicable state, with reliable transportation.
• Ability to obtain DEA licensure and collaborative practice agreement if required.
• PREFERRED: Experience in geriatrics, home health, or chronic disease management, and proficiency with electronic medical records.
