About the role
Nurse Practitioner in an outpatient Advanced Heart Failure Bridge Clinic, focusing on managing patients with advanced heart failure.
Responsibilities
- Independently evaluate, diagnose, and develop treatment plans
- Optimize guideline-directed medical therapy
- Assess volume status and initiate interventions
- Coordinate care transitions
- Educate patients and families
- Collaborate with multidisciplinary teams
- Support quality initiatives and clinical research
- Document and bill for services
Qualifications
- Master's or Doctorate in Nursing with certification as an Acute Care or Family Nurse Practitioner
- Current Maryland NP license or eligibility
- At least two years of recent cardiology experience
- Experience managing cardiovascular patients in inpatient/outpatient settings
- Strong clinical judgment, communication, and interpersonal skills
