About the role

Nurse Practitioner in an outpatient Advanced Heart Failure Bridge Clinic, focusing on managing patients with advanced heart failure.

Responsibilities

• Independently evaluate, diagnose, and develop treatment plans
• Optimize guideline-directed medical therapy
• Assess volume status and initiate interventions
• Coordinate care transitions
• Educate patients and families
• Collaborate with multidisciplinary teams
• Support quality initiatives and clinical research
• Document and bill for services

Qualifications

• Master's or Doctorate in Nursing with certification as an Acute Care or Family Nurse Practitioner
• Current Maryland NP license or eligibility
• At least two years of recent cardiology experience
• Experience managing cardiovascular patients in inpatient/outpatient settings
• Strong clinical judgment, communication, and interpersonal skills
