Cardiology APP - Heart Failure Clinic (NP/PA)

About the role

Join a specialized team in advanced heart failure care as a Nurse Practitioner (NP) or Physician Assistant (PA). This full-time outpatient role focuses on patient education, care coordination, and outcome improvement within the Heart Failure Bridge Clinic.

Responsibilities

• Evaluate and develop treatment plans for complex heart failure cases
• Assess volume status and adjust therapies accordingly
• Coordinate care transitions between inpatient and outpatient settings
• Support multidisciplinary teams in delivering comprehensive care
• Collaborate with heart failure faculty on clinical research initiatives

Requirements

• Adult-Gerontology Acute Care or Primary Care NP certification (for Nurse Practitioners) or NCCPA certification (for Physician Assistants)
• Current Maryland license or eligibility
• Prior cardiology experience required; heart failure management experience preferred
• Strong clinical judgment and teamwork skills

Schedule

Four 10-hour weekdays; no weekends or holidays.

Pay

Salary range: $58.03 to $89.94 per hour, commensurate with experience.
