About the Role
An experienced Nurse Practitioner (NP) or Physician Assistant (PA) is sought to join an Advanced Heart Failure Program within a specialized outpatient clinic. The role involves managing patients with complex heart failure, collaborating with multidisciplinary teams, and supporting clinical research initiatives.
Responsibilities
- Evaluate and manage patients referred to the Heart Failure Bridge Clinic
- Develop individualized treatment plans and collaborate on complex cases
- Assess volume status and administer interventions, including IV diuretics
- Coordinate care transitions between inpatient and outpatient settings
- Support clinical staff and serve as a liaison between various care teams
- Ensure adherence to clinical protocols and document services
Requirements
- NP: Adult-Gerontology Acute or Primary Care certification; PA: NCCPA certification
- Current license or eligibility; prior cardiology experience required
- Heart failure management experience preferred; new graduates with cardiology background considered
- Strong clinical judgment, communication, and teamwork skills
