Responsibilities

• Provide skilled nursing care to patients in the comfort of their homes.
• Develop individualized care plans and perform comprehensive assessments.
• Administer medications and treatments.
• Educate patients and caregivers.
• Coordinate care with physicians and the interdisciplinary team.
• Play a critical role in helping patients recover safely, manage chronic conditions, and avoid unnecessary hospitalizations.
• Build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient-centered care.

Requirements

• Current California Registered Nurse (RN) license.
• Current BLS certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Excellent assessment, communication, and critical thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• OASIS documentation experience.

Nice to Have Qualifications

• Previous experience working in home health, hospice, or a similar caregiving environment.
• Familiarity with KanTime Electronic Medical Record (EMR) system.
• Bilingual skills are a plus.

Pay

Salary Range: $53.00–$63.00 per hour, based on experience.
