About the role

We are seeking a compassionate Physical Therapist (PT) to join our healthcare team dedicated to delivering exceptional home health services. In this role, you will help patients regain mobility and independence through individualized treatment plans, education, and compassionate care.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety and fall prevention.
• Collaborate with physicians and interdisciplinary teams to maximize patient outcomes.
• Build meaningful relationships with patients and families while promoting independence and patient-centered care.

Requirements

• Current California Physical Therapist license.
• Current BLS certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Strong clinical assessment and treatment planning skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• OASIS documentation experience.

Qualifications

• Previous experience working in home health, hospice, or a similar caregiving environment.
• Familiarity with KanTime Electronic Medical Record (EMR) system.
• Bilingual skills are a plus.

Pay

Salary Range: $58.00–$63.00 per hour, based on experience.
