Job Summary

We are currently seeking a Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) to join our outstanding healthcare team!

Responsibilities

• Provide skilled nursing services under the direction of a Registered Nurse (RN), including medication administration, wound care, patient education, monitoring changes in condition, and documenting patient progress.
• Help patients maintain independence while promoting comfort, safety, and healing.
• Work alongside our nurses, therapists, physicians, and caregivers to achieve positive patient outcomes.
• Build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient-centered care.

Qualifications

• Current California Licensed Vocational Nurse (LVN) license.
• Current BLS certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Excellent assessment, communication, and critical thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Nice to Have Qualifications

• Previous experience working in home health, hospice, or a similar caregiving environment.
• Familiarity with KanTime Electronic Medical Record (EMR) system.
• Bilingual skills are a plus.
• Wound care/Wound Vac experience.

Pay

Salary Range: $35.00-$45.00 per hour
